A former pastor of mine understood "unworthily" to refer to our understanding of the meaning of the bread and cup. He explained the meaning at each communion so that all in attendance could partake worthily.
When I read what Paul said about this in 1 Corinthians 11, I'm struck with the impression that what he refers to as "an unworthy manner" is really eating the supper in a way that denies it to other believers.
Ironically, his statement has now been twisted into an admonition to do that very thing.
